ARC: Teaching: Tips: Peer Evaluation: Create Your Own Form

I use an online form I created for peer group evaluations. Students go to a page on our school's website and submit a form rating their peers. Instructors can either have the form data emailed to them or (my preferred method) download an Excel file containing the data.

As a solution, this is probably not the most elegant; in the Excel data file, it's easy to identify students with low evaluations but calculating averages for each student is too tedious. But it's functional, it's free, and the content (instructions, assignments and rating scale) is customizable.

The form is fairly easy to create but it requires that the instructor to has an elementary knowledge of HTML to modify the form.
